Introduction
When women think about health and longevity, they are rarely thinking about living as long as possible.
They are thinking about whether they will still feel capable in their seventies and independent in their eighties and nineties. They wonder whether ageing will gradually narrow their world, and what they can do to remain active, connected and fulfilling as the years go on.
For many of us, the story we have absorbed is that decline is simply part of getting older.
Modern science suggests that this picture is much more nuanced.
This blog is the first part of a short series exploring health and longevity through a whole-body lens. It looks at why health span matters more than lifespan, why midlife is such an important window for change, and why ageing is best understood as a joined-up biological process rather than a series of isolated problems.
Health and Longevity Are Not the Same
Health and longevity are often spoken about as though they mean the same thing. They do not.
Longevity refers to how long you live.
Health refers to how well your body and brain function during those years.
Over the last century, lifespan has increased substantially. This has been driven largely by improvements in sanitation, antibiotics and emergency medical care, rather than by prevention of chronic, age-related disease¹. As a result, many people now live longer, but spend more of those extra years managing pain, metabolic disease, cardiovascular problems and reduced mobility¹, ².
In fact, global life expectancy increased by more than 30 years during the 20th century, while the number of years lived with chronic illness rose alongside it¹. This growing gap between living longer and living well, is central to understanding health and longevity².

Healthspan: How Well You Age, Not Just How Long You Live
Healthspan describes the years of life spent in good health, with physical capability, plenty of energy, mentally sharp and with good emotional wellbeing.
Healthspan helps explain why two people of the same age can experience ageing very differently. Research using modern measures of biological ageing shows that individuals of identical chronological age can be ageing at markedly different speeds³˒⁴.
As geroscientist Dr Nir Barzilai has said, “The goal is not to live longer, but to live healthier for longer.”
When we talk about health and longevity, healthspan determines whether later years feel active and fulfilling, or increasingly restricted²˒⁴. The quality of later life is shaped by how well the body has been supported in the years leading up to it.
The diagram below shows that we are living longer. But the quality of those last 10 years, the marginal decade, depends on how well we look after our health in the previous decades.

Why Midlife Matters So Much for Health and Longevity
For many women, midlife marks a noticeable shift.
The endless energy of youth becomes elusive. Recovery from common colds & infections takes longer. Sleep lightens & shortens. Keeping weight off is more difficult. Stress tolerance narrows. Anxiety becomes more common. These changes often follow menopause and are linked to shifts in muscle mass, blood sugar regulation, inflammation and nervous system balance²˒⁵.
Research suggests that midlife is a period when women begin to diverge in how quickly they age biologically, particularly when metabolic health and inflammation are under strain⁵˒⁶. Studies using biological ageing markers show that changes during midlife are strongly associated with later-life disability, cardiovascular disease and cognitive decline⁵˒⁶.
However, midlife is not only a period of vulnerability. It is also a time when long-term resilience can be positively influenced. Midlife is a time of choice. The choices made during these years have a crucial impact on how the body functions later in life, which is why my work focuses on supporting women from midlife onwards.
A Whole-Body View of Health and Longevity
Ageing does not happen in isolated parts of the body.
It develops through established patterns that unfold across multiple systems at once.
A helpful way to think about the connectedness of the body is to imagine the body as a house, which I describe in my Healthy Ageing Video.
Each system represents a different room:
- Blood sugar and metabolic regulation: the boiler room
- Muscles, joints, bones and connective tissue: the walls and foundations
- Hormones: the communication network
- Digestion and gut health: the kitchen
- Immune system: home security
When one part of the house is under strain, others are affected. When the core systems are supported, several areas often improve together⁹.

Stress as a Driver of Health and Longevity
One factor that is often underestimated in conversations about ageing is stress.
Stress is not simply an emotional experience. It is a whole-body safety response designed to protect us.
In the short term, it raises blood sugar, increases blood pressure, puts stored glucose into our blood stream, makes us alert and increases immune messaging⁷.
The problem arises when this response is activated too often, or for too long.
Chronic stress is associated with poorer blood sugar regulation, higher inflammation, disrupted sleep and reduced recovery capacity. These same processes are consistently linked with faster ageing and a shorter healthspan⁵˒⁷˒⁸.
When we are stressed, we produce stress hormones, which in turn influence other hormones, our metabolism, our fat storage, the amount of inflammation and of course our nervous system.
All of this places extra strain on the body’s ability to regulate and repair itself over time. These effects may be subtle at first, but they accumulate over the years.
Why “Doing More” Often Backfires
Many women respond to midlife changes by pushing harder.
Eating less. Exercising more. Ignoring fatigue. Pushing through pain.
Yet long-term daily stressors, inadequate nutrition and insufficient recovery time increase the biological load on the body. This can worsen metabolic health and raise inflammation, undermining the very systems that support healthspan⁷˒⁸.
Research consistently shows that excessive training, under-fueling (meaning not enough nutrients) and poor sleep increase inflammatory and metabolic strain, particularly in midlife and postmenopause⁸˒¹⁰.
Health and longevity rarely improve through pushing-on. They respond better to professional support, nourishment and gradual, paced diet and lifestyle improvements.
Conclusion
This is the first part of a short series exploring health and longevity through a whole-body lens. Over the next few posts, we will look at how ageing is shaped not just by time, but by how well the body’s systems are supported, particularly during midlife.In the next part, we turn our attention to the early signs many women notice after 50 and what they may reflect beneath the surface. These changes often influence how you feel day to day long before they become medical diagnoses. Ageing itself is inevitable. The pace and experience of ageing are not fixed. When we understand what the body is responding to, we gain clarity. And with clarity comes the ability to make informed choices that protect strength, confidence and independence over time. Long-term evidence consistently shows that sustainable behaviours maintained over time are more protective for long-term health outcomes than short bursts of intensity or perfectionism¹⁰. Health and longevity are shaped by consistent patterns of healthy behaviours.
